In part 2 of the “Patient Safety in Primary Care Settings” series from the Nurse Practitioner Support & Alignment Network, Co-Principal Investigator Kelly M. Smith, Ph.D., will discuss the Agency for Healthcare Research and Quality’s Patient Safety Toolkit and to safely reconcile medication and engage in a warm hand-off between providers in order to ensure patient safety and continuity of care is maintained. She’ll also introduce attendees to an exclusive training cohort to implement the entire toolkit in primary care practices. This activity is approved for 1 contact hour of continuing education (which includes 1 hour of pharmacology) by the American Association of Nurse Practitioners. (Nurse Practitioner SAN)
